Landmark Hospital of Joplin has an exciting opportunity for a Social Worker to join our dedicated and professional team. Landmark Hospitals are committed to providing the highest level of customer service and quality of care to those we serve.


Summary: The Social Worker will work in collaboration with the Case Manager and other members of the Interdisciplinary Team to enhance the cost effective utilization of resources by identifying and initiating complex discharge planning to accomplish the timely transition of patients to post-hospital settings.


  • Provides assistance to patients, families, and others with a wide variety of needs to include, but not limited to, discharge planning, financial concerns, and emotional support as related to the current acute illness.
  • Provides supportive counseling to patients regarding adjustments to illness as well as other biopsychosocial needs as indicated.
  • Communicates with external agencies to provide referral information for a smooth transition from hospital to the post-hospital setting.


  • Master's degree in Social Work and LMSW or LCSW
  • Minimum of 2 years hospital experience
  • Must have an in-depth knowledge of system and community resources and referral agencies
  • Ability to communicate effectively with other healthcare agencies and practitioners
  • Excellent communication and interpersonal skills
  • Superior analytical and critical thinking skills
  • Team, detail and goal oriented
  • Positive, professional and empathetic demeanor
