Landmark Hospital of Savannah is currently looking for an experienced and knowledgeable Minimum Data Set (MDS) Coordinator to fill a position with a long-term care facility located in Savannah, GA. The ideal candidate will have a current license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) and possess at least 1 year of MDS experience gained in Skilled Nursing Facilities.

Responsibilities of the Minimum Data Set (MDS) Coordinator

  • Ensure accurate MDS completion.
  • Coordinate MDS/RAI within a transdisciplinary approach to ensure the highest degree of quality care is maintained at all times.
  • Assist with monitoring and overseeing the SNF/LTC units to assure state and federal regulations are followed.
  • Ensure utilization of interdisciplinary rehab team process in the formulation of MDS/RAI's.
  • Observe direct nursing care, review documentation and make appropriate recommendations, assist with chart audits.
  • Provide clinical support and direct to appropriate resource materials.
  • Communicate well with clients, their significant others and various other professionals to facilitate high quality client care.
  • Ensure planning and implementation of appropriate nursing assessment and care programs.
  • Assist staff, as necessary, by administering direct client care according to ANA standards and hospital nursing procedures and standards.
  • Support and adhere to existing policies and procedures (i.e., personnel, documentation, and quality assurance) on a timely basis.

Requirements of the Minimum Data Set (MDS) Coordinator

  • Current license as a Registered Nurse (RN) in the State of GA
  • Certification in MDS
  • Minimum 1 year of MDS experience gained in Skilled Nursing Facilities
  • Must Understand CMI and maximize CMI
  • Knowledge of Medicare
