Landmark Hospital has an exciting opportunity for a Case Managers to join our dedicated and professional team. Landmark Hospitals are located in Florida, Georgia, Missouri and Utah and we are committed to providing the highest level of customer service and quality of care to those we serve.

Responsibilities require your clinical knowledge every day to contribute to the overall functions of the hospital including yet not limited to Utilization Review (UR) and resource management, discharge planning, treatment plan management and financial management effectiveness.  Our case managers play an integral role in managing the interdisciplinary team approach to patient care and our goal for case managers is a maximum of 15 patients. 

Qualifications and Experience

  • Registered Nurse (RN) required
  • Three (3) or more years of work experience in a medical case management setting and confidence to manage and direct a plan of care for chronically critically ill populations
  • Management experience and/or 3 years as a Case Manager with demonstrated and effective leadership skills are required
  • A minimum of 2 years in discharge planning
  • Three years experience required in utilization review, LOS, DRG
  • A working knowledge of the insurance industry, government reimbursement, progression of care management and discharge planning skills are required
